Welcome to the Ellis Clinic!
I am Shontrice Boswell and I am excited to be a part of the Ellis School Family. I have a Bachelors of Science degree in Nursing from the University of Alabama at Birmingham. I am a Certified Pediatric Nurse with extensive pediatric experience. I am excited to be at Ellis ES full time this year. Please feel free to contact me regarding your child's health needs at school.


What Services Does the School Nurse Provide?

Health Services:
Assesses health and development, provides nursing interventions and evaluates students responses to care.

Health Education: Provides health education to students, staff and parents

Healthy Environment: Identifies health and safety concerns in the school environment.

Nutrional Services: Supports healthy food services programs.

Physical Education/Activity: Promotes health and physical education which promotes healthy behaviors.

Counseling/Mental Health: Assesses needs, provides interventions and refers students to appropriate staff or community agencies.

Parent/Community Involvement: Promotes community involvement.

Hearing and Vision Screens for KG and 3rd graders
I will be conducting hearing and vision screens for all kindergarteners and 3rd graders as mandated by the state of Virginia. This will be done for 3rd graders during the last week in September and kindergarteners will be done mid to late October. This should only take a few minutes to complete and no preparation is required. Parent volunteers are welcome. Please feel free to contact me for details.
Kindergarten Parents
Parents please have an extra complete outfit(including socks and underwear) in your child's backpack for those what if days. Unfortunately accidents do happen and a change of clothes may be needed during the course of a schoolday. Thanks parents for your attention to this matter.

Medications at school
Medications are not to be carried at school by students. All medications require a medication authorization form to be completed and returned to the school nurse with the medication in its original container. Medications will be kept in the clinic and distributed by the nurse as needed.
Emergency Cards
Parents make sure we have up to date contact information on file for all persons listed on the emergency cards. This is crucial when trying to contact parents and/or emergency contacts regarding illness or injury at school.

Attention 5th graders!
All rising 6th graders are required to have a Tdap immunization BEFORE they can enter middle school.  Please bring record of the immunization to the school nurse.  I will record the date, place a copy in the student's file, and notify the middle school.  Students will not be allowed to enter middle school, nor will lockers and schedules be assigned without record of the completed Tdap.  I am happy to answer any questions and help make this transition to middle school smooth.

Food Allergy Awareness
We have many students at Ellis Elementary School with a variety of life threatening food allergies.  Please remind your student the importance of not sharing food items during snack time or lunch time.  Also, please consider peanut and nut free snack items which will be eaten in the classrooms to help insure the safety of all students.  If your child has food allergies and does not yet have a food allergy action plan in the clinic, it can be downloaded from the link to the right.  As always, if you have any questions or concerns, please feel free to contact me.
